*There is one form for both over the counter and prescription medication.  If it is a prescription medication, it MUST have a doctor’s signature BEFORE it can be given at school.  This does include the first day of school.

Below is the current DHEC Guidance for staff/students who test positive for COVID.  

Individuals with a positive viral test.  Must stay home until the following criteria are met:

Criteria for return: 

• have been fever-free for more than 24 hours  WITHOUT the use of fever-reducing medication  

and

• Symptoms are significantly improving 

Documentation for Return- please provide 

A parent’s note that states the symptoms are significantly improving and there has been no fever in the past 24 hours without using fever reducing medication.  

If you take an at home COVID test, please complete this Attestation Form and turn it in to Nurse Ann to report your results.
